here is a transcription of his explanation for his physical health care issue:
1.On 4 23 24 my nose was broke and since 4:23/24 until now, March 2025, I've been being denied surgery to fix my nose by the Hsu at my prison and Hsu sent me to a ENT who told them to order surgery on my nose and it is almost been a year and Hsu is not ordering me surgery for now broken nose and is refusing to follow the UW hospital ENT order to give me surgery on my nose and my nose has 80% blockage  in my nose and pushed real bad to the left side of my nose with 80% of structure inside my nose on the left side of my nose.
2) on 423/24 I broke my right hand pinky finger and from 423-24 until 11:00-13:24 for 7 months I was left with a broken pinky finger on my left hand by prison Hsu and was not given any pain medication for the 7 months until I was given surgery on my left hand pinky finger on 11:13.24 can do to my pinky figure being left broken for 7 months without surgery my pinky finger did not heal properly after my surgery and got infected due to my prison Hsu not cleaning my finger causing my left pinky finger to become infected for a month from December 2024until January 2025 causing me serious pain and swelled up finger for this whole month. nurse Frederickson also lied in my Hsu files saying I infected my own pinky finger but I have Correctional Sergeant named Sgt  grosskopf  saying this nurse told me to clean my own finger and he witnessed my finger swell up and get infected  due to my prison Hsu never cleaning my left pinky finger or cared for it.
3. on 522-24 until now 2025 of March of this year I am being denied problem medical care for my asthma and chest pains and for my coughing that my prison Hsu does not treat and I wrote multiple Health Service request and wrote inmate complaints on my Hsu for denying me medical care for these medical issues and they still continue  not to treat me for these medical
I have exhausted all my inmate complaints on these denial of Medical Care issues. I have all the inmate copies of all the exhaustion documents incident reports Etc.
