Health Care advocacy history: Ploszay Tom 209964 3424 Has torn meniscus, surgeon recommended surgery, denied by provider . Afraid treatment to lower back will also be denied. "I'm trying to find an attorney to sue GBCI and this so- called provider for leaving me in constant pain for three years from a knee injury due to a construction work accident. Provider had an x ray done and said have arthritis I the knee. I said she is wrong and that something tore in the knee. Three years later she an MRI done, which showed a small tear in the meniscus. I met with an orthopedic surgeon two weeks he said he will do surgery. Now two weeks later, this provider sent a memo and refused my surgery. Said to lose weight and get a family doctor to help me when I get released on 10-29-2024. She doesn't want me any pain meds or let the surgeon give me another synvics knee injection either. There is also another clinic doctor who is supposed to come and give me three injections in my lower back due to damage and will help relieve the pain there. The GBCI provider said that the problem is arthritus but a recent MRI show that he has a torn meniscus due to a construction work accident. He saw the Prevea orthopedic surgeon who said he would do knee surgery and would also give him “Another synvics injection ”for pain. None of this is happening. When Mr Plozsay submits a blue slip requesting that the surgery be scheduled, the response is that he will see the Pain clinic June for injections in his back and tailbone. This has nothing to do with his knees. I ask you to schedule the injections and the surgery as soon as is possible. I thank you for your attention. Health care situation
So now my release date is October 29th 2024. I've been injured severely in a construction work accident before I went to prison, was crushed by about a thousand pound overhead garage door; was knocked off a ladder and crushed into a concrete floor. The door hit me in the head, neck,Back and both hands. plus my right knee hit the concrete floor which caused damage. I had multiple Specialists I had to see, as well as physical therapy three- four days a week. Went to a pain clinic and was put on several hundred pain meds a month. Had surgery on my right hand so I could get feeling back in the hand and fingers.
While being in prison for the last 8 years, I've been refused pain meds to help relieve the pain I'm constantly in Daily. I have to fight and argue with the nurses and nurse practitioner who is my provider about the pain I'm in and nothing is done. The provider gives me Tylenol and naproxen which does nothing. It barely takes the edge off the pain. My family doctor diagnosed me with fibromyalgia pain and was going to give me lyrica to see if it helps.. This prison provider here at Green Bay prison won't try me on Lyrica, Gabapentin or anything else I used to be on. I have over a three year long paper trail complaining about my pain and nothing is being done. I have sleep deprivation and nothing is done. I have headaches and back neck pain but nothing is being done. I have lower back and tailbone pain but nothing's being done. From the last time I saw the Prevea Pain Clinic Doctor Todd Rave , he said he was going to do injections in my lower back and tailbone in February 2024. My appointment to have this done isn't until June 2024. I toss and turn in bed constantly because I back hurts and goes down to my leg.
I've complained over 50 times about my back, as well as my right knee constantly hurting and that it pops grinds and gives out and feels like something tore in the. Three years later the provider finally got x-rays done. The provider said there's nothing wrong with my knee other than I have arthritis. I said that it's a lot more than arthritis and that my s y n v i c s injections only take away the popping and grinding. it took a long time to get this provider to get me in for an MRI of my knee.
On December 8th 2023 see I received a memo from Hsu at gbci from nursing staff at MRI of right knee shows a possible Shadow tear in the medial meniscus(cartilage). There is also some mild arthritis in the knee
February 21, 2024, Prevea Health Clinic orthopedic surgeon Dr Daniel Severance from in Green Bay Came to gbci prison to go over MRI and talk about doing knee. Dr Severin said he will do the knee surgery to fix the tear as well as scrape the junk that builds up and causes arthritis problems. Then after 4 or 5 months of healing he will do another s y n v i c s injection in the knee. should we try to get me in ASAP.
On February 27th 2024 I received a memo from our provider Lori Wachholz stating that the knee arthroscopy has been denied by the class III committee. said they recommend I lose weight and establish with a primary care provider as soon as possible following release from incarceration.
Now I'm supposed to live to get a job full-time getting an apartment and buy all Furnishings pay for a driver's license and buy a vehicle and auto insurance when I need knee surgery ASAP and will take at least 4 to 5 months to heal up. Then I'll have to do physical therapy for a couple months to get strength back and be able to walk and do normal activities again.
This is causing me so much stress, anxiety and depression because I don't know what to do. All I know is that I don't ever want to come back to prison, especially g b c i the way we are all mistreated in every way you can imagine.

Hello, my name is Tom Ploszay. I'm currently at Green Bay prison for party to a crime of burglary with my now ex-wife and her cousin. My ex-wife was coerced into giving statements on what storage units we broke into, in order to get a conviction on me. Cops used her medical disability to their advantage to get what they wanted. When my ex-wife gets scared she will lie over and over. My ex did this a lot to the cops. She even told the cops that we were at 8 or 9 houses and burglarized them, which my ex took these cops to show where we were at. None of these people were home, so detective left his business card for the people to call. All eight people called and said they had never been a victim of burglary or theft.
..This detective from Manitowoc didn't care that my wife at the time had lied to them about all these places. The detective had a grudge against me and only wanted to convict me. I also proved to this detective that personal property, tools for General remodeling, power tools and over $30,000 in mechanics’ tools are mine. He didn't care about the receipts or anything I had as proof of ownership. Detective listen to my wife who would say she thinks it “might” be stolen. The ex couldn't remember going to my storage units, my friends and family to get everything I owned and had in a three-bedroom house with my first wife. My ex-wife couldn't remember all the family stuff I had gotten from my mom after my dad passed away including $6,000 which I had spent a lot of the money on more tools. or she couldn't even remember the $3,000 Craftsman snowblower we got from her cousin which he had gotten from my wife's dad.
3/4 of the property of my ex-wife turned over was my lifelong personal property. The victims of the storage unit burglaries all claimed their property. All left over tools and property was mine. I even told the detective that it was all my property and no one will claim it.
The Manitowoc detective was supposed to have a court hearing about the leftover property but never did. He gave away over $30,000 Mechanics Tools power tools to their City County Garage workers. Then the sheriff's department sold all of the rest of my personal property my then wife gave to the cops. This I totaled to be between 60 and 70,000 which they got only $10,000 for.
On top of this, my then my wife and her family robbed me of everything I owned. My wife sold off two trucks,, a motorcycle, bunch of ATVs, two Snowmobiles, but most of all, she sold two of my pitbulls named k a n e and the other named Moonshine. My ex-wife will never have a driver's license or drive. She's never had anything in life other than clothes. This is why she took everything of mine. My ex also took over 60,000 worth of tattoo art and tattoo equipment.
I made a plea deal for 4 years in prison for the crimes with the storage units I broke into. Then 3 years later at the sentencing here there was an alleged victim from the storages contacted who gave a false statement for the district attorney. This alleged victim said in a police report that after looking at photos and video my property (House and garage) that I had none of his property. my wife even gave a statement that we never broke into and stole the guys’ property.
I gave a statement that my wife and her male cousin broke into the storage and others. But the detective didn't do anything. This false victim statement said he never got his property back. that my wife and I know what we were doing and that I deserve the maximum sentence and to throw away the key. My useless attorney Brian Stevens of Green Bay knew this was false information and did nothing other than tell me to shut up and save it for my appeal and I'll win.
During sentencing, the judge said he was going to go along with the four year plea deal but then after reading the alleged victim statement that he had to go along with and sentence me on what the victim wants. The judge gave me the maximum sentence of 12.5 years, giving me , 7.5 years in prison with 5 years on extended supervision plus another 9 months consecutive to the 7.5 in prison. I tried to tell the judge this was a false victim statement and he said save it for my appeal. In an appeal, the judge said he did nothing wrong and dismissed this my sentence modification. My attorney thought we would win it , saying that I was falsely sentenced for something I didn't do. I even said I would take a polygraph test to prove my wife and cousin did this and that I had nothing to do with it.

My case against the business office, inmate accounts, Judy Dinse, I like to make a carbon copy of everything I sent to her or anyone else having to do with gbi taking my money for restitution when they have been court ordered not to.
my Brown County case 16 cf-1142 Judge John z a k o w s k i who sentenced me had his judicial assistant Susan t i l o t call the prison and talk to Judy dinse to stop taking money..Judy didn't lied to me in a kite about the phone call. I then called the courts and talked to Susan. I sent judge z a k o w s k i and eight page complaint with detailed in for the dates, dollar amounts taken, and who each kite was/ to from. The judge then started a 968.-26 John Joe proceeding against Judy Dinse in inmate accounts.Once Judy d i n s e found out that a criminal investigation was being done against her, she deleted my restitution and info from the inmate trust account.

my restitution for k16 CF 1142 was paid in full on 3/12/2019. Judy d i n s e continue to take money until 12:00 7:20 21. she deleted the account to cover up how much money she took.
Then Judy DinseStarted to take restitution money for my second case 16 cf774 starting on 9/12/21. I was court ordered to make restitution payments starting when released to es from prison in the amount of 46.94 over the 36 months of ES. I had to have Madison SPD office attorney Colleen Marion bring this restitution order back into court on June 29th 2023 to amend the Joc to state that restitution not to be taken until released to ES.
Attorney Carlene then called gbci and talk to Judy dents and told her that the Joc is a court order and she needs to stop taking money. she still continued, so then attorney Colleen went back to my mom to work County judge mark wore her 4K 16 cf774 and had him write out another court order to stop taking money for this case and that I'm set up on a monthly plan once released. Judy did still continues to violate court orders and take 50% of my Hundred $400 state pay every 2 weeks ( she's statement of trust account where I highlighted). I get $1.70-1.80 Every two weeks to spend. I have to buy hygiene.
I am diabetic type 2, I also have a lot of medical problems due to a construction work accident that constantly gets unmet. I can't even get a good pair of tennis shoes to stop my feet from hurting or Gabapentin that a previous pain clinic doctor recommended to this HSU provider Lori w i c h o l z, who constantly refuses me meds and other Medical Care.
Now on this John Doe case 2023 jd5, my Judge John Zachowski ,sent the case to another Brown County judge Donald z u i d m u l d e r and the District Attorney's Office had their investigator t o d d L e p i n e investigate this. Mr Lepine wrote me to get more information. He then wrote me back saying that he contacted “the parties involved” and then told me he didn't find anything criminal was done. this is b******* and Mr Lepine never came to see me and read or copy all my paperwork. I have this proof that Judy dinse has stolen money for restitution cases that don't exist. The money of mine she has taken since March 2019 had to go somewhere. It couldn't go to a victim restitution case that was closed in March 2019 but Judy Dinse still continued to take money for the case up until 12 7:1. S o Ms Dinse deletes my account to cover up how much money she took for almost 3 years.
my Madison attorney Colleen Marion recently withdrew from the case 16 cf774 and never finished what she started with GBCI’s Judy Dinse taking money. She had to withdraw because we were also in court for ineffective assistance on my SPD attorney before Colleen, which Colleen said she couldn't take another public defender into court for ineffectiveness. So I'm waiting on a new appointment attorney Angela c h o d a k from Appleton to work on my Manitowoc case. I'm going to ask her about my Brown County case 16 CF 1142. My attorney Colleen said that her supervisor Joseph talked to her about the long letter I sent him about my trial attorney Brian Stevens of Green Bay but that's all I was told. I did write a complaint to Lawyer regulations on attorney Brown Stevens. They told me that attorney Stevens is suspended until October 8th so we can't answer any of the questions dealing with criminal cases until then.
I don't know if Lawyer Regulations can get me a new attorney and open up my Brown County case because attorney Stevens being effective throughout the entire case. Plus Stevens never put in for an appeal like I told him to do. Madison’s SPD appellate office attorneys also still haven't gotten back to me on it if they're going to open the case. The judge broke the plea deal and instead of a 4-year agreement, he gave me the maximum of 12.5 years because of this false victim statement.
I will work on this summation of my case and mail it to you this week. I have one more free stamped envel. My neighbor gave to me. he is a life sentence and no one to write
I put this order form for five hobby items I've been trying to get all year. I have large sheets of tag board and a set of Crayola colored pencils and graphite pencils so I'm a licensed tattooist. I do custom cards and like to do portraits when I can. Eventually need to get one of those 32 quart plastic containers to put my large paper and drawings on it so it don't get dirty .Any more than it is already. stuff like us to keep our our hobby stuff in one of these containers if possible.
My release MR date is 10 30 24 and I live in manitowoc. I tattoo for a living but also clean out and fix up foreclosed houses. I save all the good stuff from these houses.I take a lot of stuff to thrift stores to help them and also put a lot of stuff into a storage unit. I sell the stuff and donate 50% of the money to a church who use it to help inmates coming out of prison. The program was called “ prison after care” and was run by a woman at the Salvation Army. She passed away so all I did then was donate clothes furniture and anything I had that people from prison coming back to Manitowoc need. I still plan to do this all and get support and the program going again. Plus now that I know about your program to help inmates with stamps and hobby it, I want to donate to this program when I get out and get situated. Maybe you and I can get some people together and have an auction when I get a couple of storage units full of stuff to sell to the. Public. I have an elderly female friend who owns the storage facilities that gives me one or two large units to keep.
Aa lot of inmates getting out of prison need help with c;lothes,furniture cell phones too if they don't have money or someone to help. They may have a girlfriend and a kid who struggles and also needs help. That is why people fail when they get out of prison cuz they don't have money for the things they need at first. People go to selling drugs or doing other crimes soon as they are released just because they don't have any money. I know this because I've been revoked several times for a rule violation or hear-say and didn’t get any new charges. Then I lost everything while I was in prison and I had to start all over.
This time I actually got new charges which what I wasn't able to work for over two years due to a construction site accident, and I committed burglaries and was on a ton of pain meds. I've been locked up since 7/8/16- 7 and 1/2 years now and all I have in my release account that it shows on this account statement is a total of 47.87 to get released with. That might cover a cell phone bill if I can find a cheap place that is prepaid and will give me a free cell phone as well, like Boost Mobile. the DOC and probation officers don't care about what ex-cons have to do with. the DC is a business is only about making money off people since truth-in-sentencing started in January 1st 2000.

I fell off my steel desk stool over 2 months ago. I either bruised my tailbone or I cracked it. it's swelled up. Hurts really bad to sit, lay down sleep and walk. I roll over when sleeping Non-Stop, and I get shooting pains that wake me up from a dead sleep. All these nurses do is write things in my file and that's it. I haven't been taken to a hospital and officer said because I can still walk, and he laughed about it saying that Hsu staff are useless. So this so-called provider said she would put me on a pain med for my tailbone pain. I get real sick as well as having a bad thoughts of suicide and other problems from this med. Turns out that this med’s main use was for people with epilepsy and seizures. I eat Tylenol Extra Strength and naproxen like they are Skittles but it does no good. I need real pain meds and to go to a hospital for an MRI.
I call my mom who's in the hospital because of having a stroke. Se wishes she could help or even call the prison and Madison Bureau of Health or TV reporters but they would never understand half of what she says. The stroke damaged her speech and her right side so she can't write. She don't know what the internet is either.
Plus now this prison is harassing me with writing real Petty tickets and they just wrote me a major ticket and we're going to throw me in segregation because of a letter I mailed out to a buddy about this business office and what the woman is doing. GBCI opened my mail and never sent it out on 912/23. I got served a ticket on 10/3/23 saying I'm stalking, disrespecting, disobeying orders and lying about an employee. The lieutenant serving oke ame the ticket said he was told to put me in seg. He read the letter and said he didn't find any reason for that so he offered me 60 days loss of canteen. So now I'm going to try to go to the library and then make a photocopy and send it to my judge who started this John Doe proceeding against the business office Woman Judy Dinse. I'm not the only one who has been getting Petty tickets now and being retaliated on for speaking about this prison and some of the staff in it.
